FINANCIAL CONSIDERATIONS

1. A fee schedule is available upon request.

2. Payment is expected within thirty days of the time of the services unless insurance payment has been authorized, and a receipt will be provided.

3. Checks should be made payable to Simplified Speech Solutions. Returned checks are subject to $25.00 per item charge.

4. There may be a charge for other services, including attendance at team meetings, meetings with parents that do not include client treatment time, consultations with other professionals, preparation of reports or correspondence, any necessary court appearances, and missed appointments. Insurance does not cover these services.

5. Simplified Speech Solutions, LLC has the right to seek legal recourse to recoup unpaid balances. In pursuing these measures, only biographical information and the amount owed will be submitted in order to ensure confidentiality.

6. Session fees are set and not pro-rated for tardiness.

Health Insurance Claims

Simplified Speech Solutions, LLC directly bill various insurance companies. We will need a copy of both sides of the client’s insurance card. Most insurance companies require you to obtain pre-authorization before you begin therapy. You will be responsible for keeping track of your number of authorized sessions as well as any amount your insurance does not cover or reimburse.

Most insurance agreements require us to provide a clinical diagnosis, and sometimes additional clinical information such as a treatment plan or summary, or, in rare cases, the entire record. Simplified Speech Solutions, LLC is not responsible for any confidentiality violations incurred by insurance companies.

Cancelation Policy

In order to serve you in the best way possible and meet your needs for therapy services, the following is Simplified Speech Solutions, LLC policy on missed and cancelled appointments.

Simplified Speech Solutions, LLC requests a 24-hour notice from you if you must cancel your appointment for any reason. If this notice is not given, Simplified Speech Solutions, LLC reserves the right to charge a $40.00 missed appointment fee. Insurance does not cover missed appointment fees. The only exception to this policy is due to inclement weather. During school in-service days, consultation which is specific to the client’s programming will be provided.
